Viewing your medical record

To view your medical record in Patchs you will need to enable your NHS Online services account (see here).

If you have medical record viewing enabled, when you are signed into Patchs you should select My Records from your Patchs user menu. This is the drop-down menu in the top right-hand corner of the screen:

You will be taken to the My Records page.

The Past consultations tab will be open, showing your consultations from the past month.

Depending on what your practice has enabled, you may be able to see your:

  • Consultations
  • Problems
  • Test results
  • Medications
  • Immunisations
  • Allergies

The areas of your medical record that you can see is controlled by your GP practice, not by Patchs.

If you would like to see more of your medical record, please speak to your practice about this.

On both the Consultations and Test results tabs you can choose to display data for a particular date or across a date range.
