Creating your own questionnaires

Questionnaires can be sent to patients when you want them to answer a series of questions on a form. They are different to question sets in that they are presented to patients separate to the chatbot, and you can assign Snomed codes and scores to their answers.

Questionnaires are particularly helpful to quantify patient problems for assessment or monitoring, and can be based on your own local protocols. There are lots of existing example questionnaires in PATCHS  like the PHQ-9, GAD-7, IPSS etc that are available to all practices. You may want to create your own based on how you do, for example, chronic disease reviews or contraceptive pill checks at your practice.

You can add scores and Snomed codes for the overall questionnaire itself or individual questions. You can also trigger sending further questionnaires to the patient after they've completed one and scored above a certain threshold e.g. send the Full AUDIT questionnaire after scoring highly on the AUDIT-C alcohol questionnaire .

To create a questionnaire template for your practice, on the Create a message template page you should choose the Questionnaire type and complete all the required fields.

Any questionnaires you create will be visible to all staff at your practice but not at any other practices.
