Creating and editing your own questionnaires

Questionnaires can be sent to patients when you want them to answer a series of questions on a form.

They are different to question sets in that they are presented to patients separately to the chatbot, and you can assign SNOMED codes and scores to their answers.

Like individual questions and question sets, questionnaires act in the same way as 'Reply required' manually typed messages.

Questionnaires are particularly helpful to quantify patient problems for assessment or monitoring, and can be based on your own local protocols. There are lots of existing example questionnaires in Patchs, like the PHQ-9, GAD-7, IPSS etc. These are available to all practices.

To create a questionnaire template for your practice, go to the Create a message template page. There you should choose the Questionnaire type and complete all the required fields.

Any questionnaires you create will be visible to all staff at your practice but not at any other practices. 

Existing message templates

When you search for and select a questionnaire template, you will be able to view, edit, and delete the questionnaire.

Viewing a questionnaire

When you view an existing questionnaire, either from the Message Templates page or when you're sending a questionnaire to a patient, you will see:

  • Questionnaire settings
  • Questions tab
  • Patient view tab

Questionnaire settings

The Questionnaire title

This is the name of the questionnaire and is visible when you search for a message on the on the Contact patient page.

 Questionnaire topic for patients

The questionnaire topic for patients is the questionnaire topic that the patient sees in the chatbot message, asking them to complete the questionnaire and at the top of the questionnaire when they open it.

Questionnaire uses scores

If you enable questionnaire uses scores, you can specify a questionnaire scoring explanation. Here you can specify an explanation for score intervals.

These scoring explanations will show up when you view the answers a patient has given from the request page.

Above the patients answers, the score explanations are displayed. 

Follow up questionnaire to send

The follow up questionnaire to send on completion of this one allows you to select another questionnaire to be sent to the patient after they have completed the questionnaire.

If you have enabled questionnaire uses scores, you can set a score threshold above which to send the follow on questionnaire. This means that the questionnaire will only be sent to the patient if their questionnaire score is higher than the score threshold you have specified.

Questions

On the Questions tab you can see the question properties for your questionnaire, and these will include the question types, associated scores and SNOMED codes.

Patient view

On the Patient view, you will see how a patient view of the questionnaire, showing:

  • The patient's questionnaire name at the top (patients WON'T see the Questionnaire title that you see)
  • Dropdown boxes
  • Radio buttons
  • Free text fields

Scores and SNOMED codes will NOT be shown to the patient when they complete the questionnaire.
