This explains how to change your carer account to a patient account .
Simply follow these steps:
- Log into Patchs.
- Click on your name in the top right corner to reveal a dropdown menu.
- Select the Change My Details link.
This will load the Change My Details page.
- Here, you should choose the Create an account as a patient button.
You will be taken to the Patchs Create an account page.
- To register, first you need to click on the Patient End User License Agreement link and read the information.
- Then click Accept and Continue.
You will then be taken to the Personal Details page, where you should complete your information.
Information that is mandatory for you to enter has a * symbol next to it.
When you have finished, click the Create an account button at the bottom of the page. And you will see a message confirming that your changes have been saved.
